Job Description

  1. Plans, implements and monitors ongoing community health programs in support of the mission, values, goals and objectives of the system and local site as directed by manager or director.
  2. Provides assistance with writing and/or purchasing collateral material to support community health initiatives.
  3. Represents local site and/or UMMS at a variety of local health coalitions or with external community partners.
  4. Works on planning and development of projects with a variety of internal and external partners.
  5. Assists in the development and implementation of community health programs for targeted community population.

Qualifications

  • Bachelor’s Degree preferred or at least 1-2 years experience and training in a health related field – i.e. health education, nutrition, exercise science.
  • Two (2) to five (5) years of related or internship experience preferred.
  • Current CPR experience and/or Emergency First Aid training preferred.

2. Ability to work well in a fast-paced environment. 
3. Able to manage multiple tasks and projects concurrently; exceptional organizational skills.
4. Highly effective verbal and written communication skills including editing skills are expected.
5. Ability to review data and determine trends. Ability to assist in developing action plans based on the data.
6. Ability to enter data into a database – either CBSA and/or Excel to analyze program outcomes and program 
reach.
7. Microsoft Office experience and some medical terminology
8. Ability to communicate effectively both verbally and in written form with the patients, physicians, health 
system officials, other health system personnel, vendors and the general public is essential.
9. Valid driver’s license required.
10. May need to work some weekends. 
11. Access and use of personal vehicle for travel purposes your information will be kept confidential according to EEO guidelines.
