Summary of Duties

As a Case Manager, you will:

This position assesses Oregonians applying for, or currently receiving, long-term care services to develop, coordinate, and arrange a variety of services to allow older adults and people with disabilities to get the continuity of care they need. Case Managers provide choice counseling and help establish, develop, and implement case management plans, coordinate the provision of services for in-home, community-based care and nursing home care, regularly review and update long-term care service eligibility, monitor social service plans, and identify and mitigate safety risks.

  • Provide case management by coordinating the provision of long-term care services to Oregonians according to rules, regulations, and the individual case plans.
  • Contact Oregonians or collateral contacts monthly to set up and monitor service plan and safety needs. This could include in person, the phone, or electronic contact.
  • Assure that service delivery is provided in a culturally competent and trauma-informed way; assure that printed materials are available in different languages/formats; bilingual services available and facilities are accessible for all Oregonians.
  • Work in collaboration with the eligibility team and utilize the ONE System to review and update Long Term Care Services, along with reviewing and providing information to Oregonians.
  • Interview Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective service needs. This can include collateral interviews both in person and on the phone. Develop detailed, individualized, and person-centered case planning for Oregonians.
  • Conduct interviews in person/on-site using the Case Assessment and Planning System (CA/PS) assessment tool on a state-issued laptop. 
  • Process applications, inquiries, and data according to policy timeframes, ensure appropriate documentation and forms are completed and sent, and communicate eligibility decisions to ensure delivery of benefits and services.
