About the role:
The Clinical Liaison serves as the clinical and relational bridge between the organization and referral sources. This role is responsible for building trusted relationships, educating providers and facilities on hospice eligibility and services, and supporting appropriate, timely hospice referrals. The Clinical Liaison combines strong clinical knowledge with relationship management to support census growth while upholding the highest standards of ethical and compliant practice.
Job Responsibilities:
Referral Development & Relationship Management
- Develop and maintain strong relationships with hospitals, skilled nursing facilities, assisted living communities, physician offices, and community partners.
- Serve as a visible, trusted clinical resource to referral sources.
- Collaborate with Business Development and Operations teams to support census growth within assigned territories.
Clinical Education & Support
- Educate referral sources on hospice philosophy, services, eligibility criteria, and levels of care.
- Provide guidance on appropriate hospice referrals while maintaining compliance with CMS Conditions of Participation.
- Participate in care conferences, in-services, and community education events.
Referral Management & Coordination
- Respond promptly to referral inquiries and coordinate with intake and admissions teams.
- Review clinical information to assess hospice appropriateness and support smooth transitions of care.
- Ensure accurate and timely documentation related to referral activity.
Compliance & Ethics
- Adhere to all federal, state, and organizational compliance standards.
- Conduct all referral and relationship-building activities ethically and in alignment with hospice regulations.
- Maintain patient confidentiality and HIPAA compliance at all times.
Collaboration & Reporting
- Partner closely with interdisciplinary team members, leadership, and marketers.
- Track referral activity, outcomes, and relationship engagement using CRM or tracking tools.
- Participate in team meetings, training, and ongoing performance improvement initiatives.
Job Qualifications
- Current, unrestricted RN license (preferred) or other clinically licensed professional as permitted by state regulations (e.g., LPN, LMSW).
- Minimum 2–3 years of hospice, palliative care, or acute/post-acute clinical experience.
- Strong understanding of hospice eligibility, levels of care, and referral processes.
- Excellent interpersonal, communication, and presentation skills.
- Valid driver’s license with reliable transportation.
