Job Description Summary:

Under general supervision, the Behavioral Health Crisis Response Clinical Case Manager provides connection, support, and referral to resources and treatment for community members following contact with Behavioral Health Crisis Response Team (BHCRT) clinicians. Case managers help to stabilize people following a crisis and prevent future hospitalization whenever possible by connecting people with ongoing support in the community. Case managers collaborate with the larger team of case managers and clinicians and are part of the team’s alternative response model. Alternative response involves responding to calls with other BHCRT staff that are lower in acuity than calls dispatched to clinicians and Boulder Police Department officers. The primary focus of the clinical case manager is supporting community members who are falling through system gaps, connecting them with resources, and preventing future crises.

Job Description:

ESSENTIAL DUTIES AND RESPONSIBILITIES                 

  • Coordinates with Behavioral Health Crisis Response Team to connect with clients following community contact or referral from Public Safety personnel.
  • Provides short term case management to high-risk community members to support access to available resources, benefits, and care.
  • Provides in-person response, along with other BHCRT staff, to situations involving behavioral health concerns or crisis in a variety of community settings via Boulder Police & Fire dispatch, including homes, businesses, street corners, parks, congregate living facilities, and shelters.
  • Advocates for appropriate access to care and/or resources while supporting clients’ strengths and resiliency.
  • Meets with clients and community members to determine case management needs and facilitate care coordination with appropriate providers. May involve intensive in-home or community-based contact.
  • Clinical case managers transport clients to medical appointments, food pantries, benefits offices, and other locations as needed.
  • Communicates, collaborates, and creates relationships with multiple collateral agencies, including resource providers, county services, schools, hospitals, family members, and significant others to facilitate referrals for needed community-based services and treatment.
  • Participates in clinical supervision and team meetings according to team policies. 
  • Documents all contacts according to team policies.

MINIMUM QUALIFICATIONS

  • Ability to communicate ideas clearly in written or spoken form, particularly as it applies to documentation, care coordination, and in crisis situations. Completes work in a timely and consistent manner in accordance with team policies. 
  • Ability to communicate tactfully and effectively with the public and colleagues. 
  • Ability to demonstrate emotional regulation during interactions that may involve distressed or antagonistic individuals and manage secondary trauma related to interactions with emergency and crisis systems. Ability to demonstrate knowledge appropriate to education and experience of major mental illness, personality disorders, traumatic brain injury, dementia, complex trauma, intellectual/developmental disabilities, substance use disorders, trauma informed care, harm reduction, and de-escalation.
  • Ability to demonstrate knowledge of community resources and make appropriate referrals to those resources as well as ability to navigate insurance benefits and associated applications.
  • Ability to demonstrate proficiency in Microsoft Office suite and the records management systems used by the team.
