JOB SUMMARY

The Youth Care Team (YCT), within the Philadelphia Department of Public Health’s Division of Reproductive, Adolescent, and Child Health (ReACH), utilizes a multidisciplinary, youth-centered approach to support adolescents in achieving their sexual, reproductive, and mental wellness goals. YCT provides education, resource navigation, and referral support through partnerships across schools, community-based organizations, and non-clinical settings.

The Education & Partnerships Coordinator plays a key role in advancing YCT’s strategy to expand access to adolescent health education and resources across Philadelphia. This position is responsible for leading partnership development efforts and coordinating the delivery of high-quality, skills-based health education in youth-centered settings, including schools and Free Library of Philadelphia branch locations.

This role requires a high level of initiative, adaptability, and strategic thinking to identify gaps in access, respond to emerging adolescent health needs, and ensure programming remains relevant and impactful. The coordinator will contribute to strengthening YCT’s presence across systems and support the evolution of programming to meet the changing needs of youth.

JOB SPECIFICATIONS

Responsibilities/Duties


Lead the identification, development, and management of partnerships with schools, library branch sites, and community-based organizations to expand YCT services.

Coordinate and oversee the implementation of YCT programming across partner sites, ensuring consistency, quality, and alignment with program goals.
Facilitate engaging, skills-based educational sessions for middle and high school-aged youth on sexual, reproductive, and mental health topics.
Develop, adapt, and refine lesson plans and educational materials to reflect current adolescent health trends and emerging needs.
Assess gaps in access to adolescent health education and proactively identify new opportunities for expansion.
Build and maintain strong, collaborative relationships with school staff, librarians, and community partners.
Monitor trends affecting adolescent health and contribute insights to inform program direction and priorities.
Provide youth with education, resources, and referrals in an affirming, confidential, and non-judgmental manner.
Collaborate with YCT staff to align education, outreach, and systems-level strategies.
Track partnerships, program delivery, and youth engagement; ensure accurate and timely reporting.
Contribute to program planning, evaluation, and continuous quality improvement efforts.
Maintain confidentiality and adhere to all relevant regulations and guidelines.
Attend meetings and training courses as required.

Qualifications

Education


Bachelor’s degree required; degree in public health, education, social work, human sexuality, or related field preferred.

OR

Equivalent relevant combination of professional and lived experience.

Experience/Skills

3–5 years of experience in public health, adolescent health, education, community outreach, or a related field, including experience facilitating educational programming for middle and/or high school-aged youth.
Strong interpersonal skills, with the ability to build and maintain relationships with diverse stakeholders, including youth, community partners, and providers, and demonstrated ability to build and sustain partnerships across schools and community-based settings.
Experience developing or adapting curriculum, lesson plans, or training materials, and delivering engaging, youth-centered education.
Strong organizational and project management skills, with the ability to manage multiple priorities, track program or grant activities, and produce accurate and timely reports.
Ability to work independently with a high level of initiative, accountability, and follow-through, while contributing to a collaborative team environment.
