Responsibilities:

• Participate in Connecting NJ initiatives and workflows to support overall program reach
• Case Manager will receive referrals from the Connecting NJ HUB for families who require additional resources and follow up.
• Will work closely with Early Childhood Systems of Care and existing resources to help link families to the most appropriate and available services, with special emphasis given to families requiring a Plan of Safe Care.
• Provide care coordination for families, research most appropriate resources and provide advocacy and follow-up as needed.
• Responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client centered
• Assists with identifying gaps and barriers to receiving services, and alleviate system level concerns and trends families experience with service utilization.

Requirements

• Experience in maternal-child health (MCH), infant/early childhood mental health, parent/family support, and/or related fields and settings.
• Bachelor’s degree required, preferred in Psychology, Social Work, Mental Health Counseling, Early Childhood Education or related education
• Bilingual - Spanish fluency
• Temporary coverage of other counties may be required when counterpart is unable to attend meetings or contact clients

Additional Information:

• Hourly Rate: $21.57
• Bilingual Rate: Additional $1.50/ hourly rate 
• Schedule: Monday-Friday, 9a-5p
• Sign-on Bonus: $1000
