Overview

Registered Nurse, HomeCare 

$5,000 Sign-on Bonus for Experienced RNs

Additional $1,500 Sign-on Bonus for candidates with relevant HomeCare Homebase experience

Full Time, 80 Hours Per Pay Period, Day Shift

Weekdays 9:30 am to 6 pm

Position Summary:

Provides direct support for home health and hospice clinical field staff supervisor (CFSS) that promote efficient delivery of quality patient care.

Responsibilities

  • Receives clinical calls for home health and hospice service lines and documents calls and action taken.
  • Triages clinical calls by conducting an initial telephone assessment, discuss recommended action based on plan of care with the caller and completes documentation in the clinical record. Communicates with physician as needed to resolve issue/support patient needs.
  • Receives and documents physician orders in the clinical record per policy and communicates with patient(s) and appropriate caregiver(s) as needed.
  • Co-signs and process physician orders in electronic medical record.
  • Completes designated workflow assignments timely and accurately.
  • Serves as resource for visiting staff dealing with medication questions/interactions and education.
  • Receives complaints and completes follow-up timely. Documents and communicates to clinical supervisor(s) and other appropriate persons.
  • Completes assigned workflow as designated by Clinical Supervisors.
  • Provides excellent customer service to both external and internal customers.
  • Performs patient visits in times of need.
  • Provides coverage for clinical field staff supervisor position as needed during absences.
  • Local travel required.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.

Qualifications

Minimum Education:           

Graduate of accredited school of nursing with diploma, ADN, BSN or MSN

 

Minimum Experience:         

Four (4) years professional experience.

 

Licensure Requirement:      

Must have and maintain Tennessee Registered Nurse (RN) licensure. Employee must have a valid Tennessee driver’s license and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure. CPR required
