About our Gender Affirming Hormone Care and Behavioral Health Patient Navigator position

Working closely with the Gender Affirming Hormone Care (GAHC) Program Director, the GAHC Program Navigator participates in the advocacy, outreach, intra and inter-departmental collaboration, and follow-up/patient navigation aspects of PPGHNAIK’s GAHC program.

The GAHC Program Navigator provides patient navigation services for GAHC patients in the six states of the affiliate, provides referral coordination, insurance navigation and other support services for patients accessing GAHC services.

The Navigator provides and coordinates information and resources to PPGNHAIK staff in support of patients, as well as direct patient communication and support. The GAHC Program Navigator works with the clinical staff to assist in the provision of affirming clinical care.

This role works with the Advocacy and Education departments to conduct community outreach and promote the program and advocate for the rights of transgender, non-binary and intersex individuals. The GAHC Program Navigator will work closely with clinical staff to remove barriers to care that would otherwise prevent or hinder patients from receiving health care services.

What will you be doing!
The GAHC Patient Navigator provides referral coordination and facilitation for PPGNHAIK patients accessing GAHC services.

Understands prior authorization requirements for insurance, insurance coverage challenges, and assists patients in accessing insurance.

Collating community resources, building PPGNHAIKs referral network, and maintaining up-to-date materials related to social services, medical care, and other needs in our service area.

Point of contact for administrative functions related to the provision of PPGNHAIK’s GAHC program.

Serving as a resource for GAHC studies with PPGNHAIK’s Research department.

Key Responsibilities:
Coordinating administrative aspects of the transgender health care services program in accordance with PPGNHAIK’s Medical Standards and Guidelines and Policies.

Working collaboratively with staff to respond to and complete prior authorizations.

Tracking new patient requests, provide monthly statistics and referral patterns.

Providing patient follow–up and works closely with the Centralized Follow-Up team, assisting with patient calls, communication, and follow-up plans.

Serving as an ongoing resource for Patient Access Center staff. Supports Patient Access Center staff in answering questions from prospective or current patients regarding gender affirming hormone care that are beyond the scope of appointment setting or easy to manage referrals.

Returning all phone calls and other communications from patients and non-patients regarding resources and services. Tasks any clinical needs directly to clinicians.

Assisting patients and providers in preparing letters for gender marker change, name change and surgery.

Assisting patients and providers with pre-authorizations and insurance denials.

Maintaining and expanding PPGNHAIK’s GAHC resource and referral lists, collates community resources, builds the referral network, and maintains up-to-date materials related to gender affirming services, care, and ancillary needs.

Coordinating with Marketing to ensure that PPGNHAIK is well represented in community events, tabling, conferences, etc.

Working in conjunction with the Research Department to manage research projects related to GAHC and recruiting interns to support these projects and the GAHC program.

Minimum Qualifications:
High school diploma required.

Bachelor’s and/or graduate degree in related fields (counseling, social work, and chemical dependency) preferred.
