Who We’re Looking For
You are an Clinical Care Coordinator who is passionate about helping individuals suffering from substance use disorders and/or mental health illnesses. You are someone who realize that the disease of addiction doesn’t discriminate, and can therefore work with individuals from diverse backgrounds. You have the ability to treat those in treatment with respect and dignity, by providing quality services. You are trust worthy, empathetic, adaptable, compassionate and a great listener. You are a problem solver and is not afraid to have difficult conversations. You have the ability to positively influence the vulnerable. You have good interpersonal and collaboration skills, with the ability to work successfully with people at all levels. You want to join an experienced, supportive team who is also passionate about helping individuals suffering from addiction and mental illness, achieve a better quality of life, sobriety, and abstinence from substance use/abuse. You understand that while the industry is demanding, your role is significant to helping our clients reach and sustain sobriety, thus are flexible and willing to occasionally work a rotating weekend shift as needed. What You'll Do At Gaudenzia
- Responsible for conducting intake interview and orientation to care coordination, with client prior to discharge from residential level of care.
2. Responsible for liaison with clinical staff for review and verification of recommended services, referral to clinically recommended level of care, and collateral contact information.
3. Completion of Recovery Plan in coordination with client and primary clinical counselor.
4. Responsible for identifying health care and recovery support needs with client and initiating referrals.
5. Responsible for assisting clients in getting to the next level of care (if applicable) by removing barriers, providing reminders, and conducting outreach when appropriate.
6. Responsible for developing and maintaining relationships with services and programs. These linkages will include but are not limited to treatment, health care, employment, legal, housing, child welfare, social services, peer-support providers, etc.
7. Establish and maintain care coordination services and contact schedule.
8. Maintains client records and completes all other required documentation.
Qualifications for This Role:
D.C. Education/ Licensure Requirements:
Education and Training: A minimum of an Associate Degree or full certification as an Addictions Counselor. Employee must have successfully completed annual training requirements of 25 hours or more per year. For DC must possess the following licenses or certifications: LICSW or LPC
Experience:
- Associates Degree: Five (5) years
- Bachelors Degree: Three (3) years
- Masters Degree: Two (2) years
- Certified Addictions Counselor (CAC): Five (5) years
