• The initial compensation for this position ranges from $69,970.86 - $86,055.33 per year.
• Salary is dependent on commensurate experience above the minimum qualifications for the role and internal equity considerations.

Who We Serve 

Pacific Clinics serves children, transitional age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.  

POSITION SUMMARY:

The Licensed Clinical Access to Care Coordinator monitors and coordinates all referrals to assigned programs ensures appropriate access to care for consumers. Conducts quality assurance reviews of intake documentation. Assists with facilitating the transfer and referral of consumers to outside resources and service providers. Provides individual, group and/or family therapy to clients and/or families as needed. Provides clinical assessment, field-based and outreach services to clients and their families; case management and advocacy services as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

•  Works with Pacific Clinics Call Center and the Clinical Quality Assurance Access to Care Specialist to track referrals and Access to Care outcomes. Tracks trends such as no-show rates to assist with scheduling.
•  Monitors intake slots regularly to facilitate availability and ensure that language capabilities meet referral needs.
•  Collaborates regularly with the Pacific Clinics Call Center staff to ensure the scheduling process is running smoothly.
•  Reviews intake documentation for diagnostic accuracy and ensures critical elements are included such as the presence of a substance use disorder, culture and high risk issues.
•  Reviews intakes after completion for level of care and assists with transfer process both internally and externally.
•  Assists with continuous quality improvement efforts regarding intake process and chair meetings as required
•  Assists with transfers, both within Pacific Clinics and to outside providers, during the treatment process.
•  Collaborates with case managers and other staff to maintain a list of outside referrals sources for mental health care.
•  Communicates and collaborates with Los Angeles County Department of Mental Health Service Area Navigators as appropriate.
•  Assists with case assignment during disposition and track updates in Welligent.
•  Assists the program director with reviewing assessment and re-assessment checklists regularly and update as agency changes implemented.
•  Conducts intakes not assigned to other staff and act as back-up as available during staff absences.
•  Completes Access to Care triage screening tools with walk-ins as available.
•  Conducts family-centered, strengths-based, culturally competent individualized intakes/assessments gathering information from family, consumer, significant other and  involved agencies in the office or in the field as appropriate.
•  Includes healthcare, substance abuse, co-occurring disorders, employment or housing, as appropriate, in treatment plans.
