Job Summary:
The Licensed Clinician is a core member of the Collaborative Care team involving the patient's primary care provider, psychiatric consultant, and other behavioral health providers. The Licensed Clinician provides evidence-based interventions using the Collaborative Care Model to promote system reduction and optimal patient outcomes. They use a world-class technology platform to provide brief interventions using evidence-based techniques such as Problem Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation in Internal and Family Medicine setting.

- Base salary is factored on licensure level, geographic location, and budgets paid on a biweekly basis
- License Clinician requiring supervision located in the state of Connecticut: $62,700 - $78,100
- License Clinician that does not require supervision located in the state of Connecticut: $75,900 - $85,800
- Performance driven incentives can range from $860 to $2,200 based on data supportive targets paid on a monthly basis
- Entry into the plan will begin on the first of the month following three months of completed service (month 4)
- You will receive a $500/month stipend for your first three months of employment.
- To ensure all employees are aware of their performance against their targets, and have full visibility into their variable compensation, regular 1:1 meetings will be required with their direct supervisor

- Monday - Friday: 8:00am - 4:00pm
- Monday - Friday: 9:00am - 5:00pm
- Monday - Friday: 10:00am - 6:00pm
- Monday - Friday: 11:00am - 7:00pm
- Monday - Friday: 12:00pm - 8:00pm
- Master's Degree in Social Work from an accredited program.
- Licensed to practice in Connecticut as an independent social worker (LCSW, LICSW, or equivalent) OR
- Licensed to practice in Connecticut as an associate social worker under supervision (LCSW, LMSW, or equivalent) OR
- Able to complete cross-licensing to Connecticut within three months of employment. Concert Health will cover the cost
- Bilingual in English and Spanish
- Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation)
- Expertise in handling higher-acuity patients/situations
- Technical proficiency in a variety of platforms including EHRs, ability to quickly learn new technology
- Knowledge of/experience with behavioral health screening tools (PHQ9, GAD7, CSSRS, etc)
- Milestone One: Qualified candidates will be scheduled for a 30-minute virtual interview to discuss their experience, qualifications, base salary expectations, and an overview of Concert Health’s Collaborative Care Model
- Milestone Two: Qualified candidates will be scheduled for a 30-minute assessment screen analyzing the competencies required for the role
- Milestone Three: Qualified candidates with the required experience, qualifications, and base salary expectations will be scheduled for a 45-minute virtual interview with Clinical Leaders
