Pharmacy Medication Reconciliation Technician

Job Description:

• Pharmacy medication reconciliation technician is a certified pharmacy technician who supports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital.
•  Information is obtained by interviewing the patient, patient family, calling pharmacies, long term care facilities and/or other providers.
• The information obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital
• Transitions of Care pharmacists will serve as subject matter expert for their area of care.

Requirements:

• High school diploma or GED Preferred but not required
• 1-year related work experience preferred but not required
