Registered Nurse (RN) – Home Health (Full-Time, Field)

Location: Dallas, TX
Schedule: Full-Time | Field-Based
Reports To: Director of Nursing / Clinical Supervisor

About the Role

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team in the Dallas area. The ideal candidate will provide high-quality, patient-centered nursing care in the comfort of patients’ homes. This position offers autonomy, flexibility, and the opportunity to make a direct impact on patients’ health and quality of life.

Key Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans in collaboration with the interdisciplinary team.
  • Provide skilled nursing care including wound care, IV therapy, medication administration, and disease management education.
  • Monitor patient progress, document clinical findings, and communicate changes in condition to the physician and care team.
  • Educate patients and caregivers on self-care techniques, treatment plans, and medication management.
  • Coordinate with physical therapists, occupational therapists, social workers, and home health aides to ensure continuity of care.
  • Maintain accurate, timely, and compliant documentation in accordance with Medicare, state, and agency guidelines.
  • Participate in case conferences and staff meetings as required.
  • Ensure compliance with infection control, safety, and quality standards across all home visits.

Qualifications

  • Current RN license in the state of Texas (required).
  • Minimum of 1 year of nursing experience; home health experience preferred.
  • Valid driver’s license and reliable transportation (required).
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, organizational, and documentation abilities.
  • Proficient with EMR systems (experience with Homecare Homebase preferred).
  • Ability to work independently and manage a field caseload efficiently.
