What We Offer

The City and County of Denver offers a competitive salary commensurate with education and experience. The salary range for this position is $23.33 - $30.00. We also offer generous benefits for full-time employees which include but are not limited to:

Location

The City and County of Denver supports a hybrid workplace model. In this position, you can expect to work mostly in the field, and within the office when needed. The office for this position is located at 201 W. Colfax Ave., Denver, CO 80202.

What You’ll Do

Denver's Department of Public Health and Environment (DDPHE) is dedicated to advancing Denver's environmental and public health goals. DDPHE works collaboratively with city, state and community partners to conduct education, community engagement, and enforcement to promote healthy people, healthy pets, and a sustainable environment. DDPHE oversees a number of services related to public health and the environment and is comprised of seven divisions: Division of Shared Services and Business Operations; Community & Behavioral Health; Public Health Investigations; Denver Animal Protection; the Office of Medical Examiner; Emergency Management and Medical Operations; and Environmental Quality. As the Outreach Case Coordinator you will be working to fulfill one of Mayor Johnston’s goals of ending unsheltered homelessness in Denver.  You will be working and collaborating with a multidisciplinary team from Denver Department of PUblic Health and Environment and other city agencies.  You will provide outreach, resources, and referrals to people from various backgrounds and experiences who may often experience trauma, stigma, oppression; people who use substances, people living with mental health challenges, and people experiencing unstable housing.  The majority of the workday will be spent outside, in various parts of Denver, in all weather. 

Specifically, as the Outreach Case Coordinator you will:

  • Work daily within a geographic area prioritized for outreach to connect people to shelter, housing, and other services.

  • Collaborate closely with multidisciplinary outreach team to develop daily operational plan focusing on unique demographics, service needs, and other special considerations.

  • Approach individuals on the street in the Focus Area.  Engage individuals in trauma-responsive, person-focused interactions, letting them know you’re there to help.

  • Assess housing status and needs of those in focus area

  • Provide support and resource navigation to those seeking services, including connecting individuals to Behavioral Health services and shelter

  • Transporting individuals to their next destination to facilitate a face-to-face transfer to the next service provider

  • Ensure vehicle is fully stocked with supplies necessary for successful operations and track overall usage of supplies and inventory

  • Provide comprehensive support and accuracy in relation to data needs and collection

  • Maintain documentation standards in recording daily activities and interactions

  • Along with all DDPHE staff, actively participate in emergency preparedness and response team activities to support DDPHE’s responsibilities outlined in the City’s Emergency Operations Plan. This may require being ‘on-call’ from time to time respond to emergencies. Training will be provided.

  • Other duties as assigned.
