Licensed Medical Social Worker

Pay: $75-$90 per visit with mileage reimbursement (dependent on distance willing to drive between clinics)

Schedule:

• RPN- Part-Time
• Hours / Days-M-F
• Weekends? As needed, not normally
• How many hours expected to work per week? 1-5 per week

Role Overview:

We are seeking a motivated MSW with experience in home health care to join our team in Colorado. In this role, you will provide MSW services to our medically fragile patient population across the front range. This includes both adults and pediatric clients and their families.

Requirements

Key Responsibilities:

• Assess psychosocial status and communicate findings.
• Conduct social evaluations and plan interventions.
• Maintain clinical records.
• Provide information and referral services.
• Serve as a liaison between patients/families and community agencies.
• Collaborate with agency personnel.
• Develop contracts with agencies as resources.
• Participate in training programs.
• Contribute to plan of care and case conferences.
• Assist in understanding social factors related to health problems.
• Participate in discharge planning.
• Perform other delegated duties.

Qualifications:

1. Must have an active MSW license with the state of Colorado.
2. Must be experienced working with both adults and peds/family dynamics.
3. Minimum of one year's experience in health care
4. Possesses and maintains current CPR certification.
5. Must be a licensed driver with an insured automobile in good working order.
