In this dynamic role, you will monitor clients' health, conduct thorough assessments, and maintain essential communication with support teams and agency management. Key Responsibilities:
· Conduct and document monthly head-to-toe assessments of residential clients in their homes.

· Maintain communication with agency management and the client’s support team, documenting all changes in health status and needs.

· Ensure staff supervision and specialized instruction are conducted and documented as necessary.

· Complete monthly documentation for activities related to the role and ensure annual authorized units are delivered.

· Attend training sessions and meetings as required, including Level 1 Med Aide Instructor duties if assigned.

· Assist the Regional Director in responding to relevant reviews and adhere to agency policies and procedures.

Education, Experience, and/or Credential Qualifications:
Must be a currently registered, licensed registered nurse in the State of Missouri.

Additional Qualifications:
· Successful completion of a background check including criminal record, driving record, abuse/neglect, and fingerprint check.

· Current driver's license, acceptable driving record, and current auto insurance.

· Must have and maintain current Missouri RN license in good standing.

· Certification in CPR and First Aid is required.

Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during the work shift.

Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
