We are seeking a dedicated and detail-oriented Staff Pharmacist to join our Anticoagulation Clinic team. The candidate will be responsible for managing and optimizing anticoagulant therapy for patients, focusing on medications such as Warfarin, Enoxaparin, and Direct Oral Anticoagulants (DOACs). Key responsibilities include:-Providing medication counseling to patients on anticoagulation therapy.
-Monitoring and adjusting Warfarin dosing based on INR levels.
-Managing Enoxaparin therapy, including patient education and monitoring for proper use.
-Assessing and transitioning patients to DOAC therapy if deemed clinically safe and appropriate.
-Collaborating with healthcare providers to ensure optimal therapeutic outcomes and patient safety.
-Educating patients on the importance of adherence to anticoagulation regimens and potential side effects.
-Maintaining accurate records of patient treatments and progress.
-Leading and participating in workflow improvement initiatives to streamline clinic processes, reduce wait times, and enhance patient care delivery.
-Identifying and implementing best practices for medication management, documentation, and patient monitoring.
-Monitoring clinical outcomes and providing feedback for continuous improvement in the delivery of anticoagulation services.
-Supporting data collection, analysis, and reporting to measure the impact of workflow changes on patient care and clinic efficiency.
Job Description:
EDUCATION:
- Doctorate: from an accredited college of pharmacy or equivalent education/experience
CERTIFICATION & LICENSURE:
- PHARMD-Licensed Pharmacist
TYPICAL EXPERIENCE:
- 3 years recent relevant experience .
SKILLS AND KNOWLEDGE:
- Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations and laws and regulations.
- Knowledge and understanding of different Pharmacy practice settings, including narcotic delivery and procedures.
- Knowledge and ability to identify and employ pharmaceutical and medical terms, abbreviations and symbols commonly used in prescribing, dispensing, and record keeping of medications.
- Requires a basic working knowledge of legal requirements and accreditation standards including The Joint Commission, Title XXII, Department of Homeland Security (DHS), Drug Enforcement Administration (DEA), Food and Drug Administration (FDA), and United States Pharmacopeia (USP).
- Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
- General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), related pharmaceutical technology, Electronic Health Records (EHR), and EPIC.

Pay Range is $95.53 to $105.07 / hourThe salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
