Sangre de Cristo Community Care is seeking a full-time Registered Nurse to join our dynamic Home Health team in La Junta, CO. $5,000 Sign-On Bonus
Starting Pay: $38–$43/hour (based on experience)
Minimal On-Call – Dedicated Weekend Coverage Provided

This is a Full Time Benefited position. Monday - Friday, 8-5.

Responsibilities:
· Plans, coordinates, implements, and evaluates the nursing care of all patients.
· Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs
· Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
Requirements
- A valid non-restricted RN license in the state of Colorado is required.
- Must have a valid non-restricted driver’s license.
- Flexibility and availability to take call for home health and hospice patients.
- At least 1 year of home health or hospice experience preferred.
