Tufts Medicine Care at Home is seeking a Hospice Liaison Admission RN who will work under the direction of the Referral Management and Transitions of Care Manager and the Director of Referral Management and Transitions of Care. This position is responsible for patient assessments and GIP admissions to Hospice in the hospital setting while ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team.

As a key member of the health care team, the Hospice Liaison must interact courteously and effectively with patients and their families, as well as with co-workers from all Agency departments, community resources, and with patients' physicians. The Hospital Liaison carries out their responsibilities consistent with Agency policies, mission, and vision.

Location: Lawrence Area

Hours: Monday-Friday 10AM-6:30PM

Essential Functions

  • Collaboratively works closely with the LAGH hospital care teams and community, identifies, establishes and maintains positive working relationships with all facility contacts.  
  • Collaboratively works with care teams to identify patients potentially hospice appropriate.
  • Evaluates patients for potential hospice care needs and determines if hospice benefit status is appropriate, as requested by hospital clinical staff, or physicians.
  • Responsible to guide the patient and family through the admission process.
  • Understands and stays current regarding hospice conditions of participation, reimbursement options and criteria for services.
  • Responsible for hospice in-service education programs as assigned.
  • Responsible for facilitating family meetings, obtaining consent/election form signatures and gathering medical data for review by Hospice physicians.
  • Dependent on volume may complete hospice admission and/or outreach in the greater Melrose Wakefield community, if needed.
  • Seeks opportunities to interface with referring and non-referring health care professionals, patients, families and the community, to promote Tufts Medicine Care at Home.
  • Collaboratively works with agency staff to ensure coordinated hospice discharges or admit GIP at the hospital setting environment, if appropriate. Responsible for Routine LOC admissions in the community.
  • Arranges for durable medical equipment, and medical supplies needed to meet patient care needs.
  • Completes required paperwork and data entry into electronic medical record by the end of each business day.

Position Type/Expected Hours of Work & Travel

This is a Full-time day position, either (4 ) - 10 hour shifts OR (5) - 8 hour shifts, and rotating weekends.

Qualifications

  • Active RN licensure in Massachusetts and New Hampshire
  • Previous experience in hospice and/ or admissions preferred.
  • Must have a current valid driver’s license and the ability to travel to key accounts within the agency’s service area.
