Job Summary:

RN support is required for dressing changes and lab draws are needed for this patient population. They will support these providers in patient assessment, triage phone calls, and work message baskets that support a program volume of about 20,000 visits per year and 5,000 sedated and non-sedated physician procedures. This includes supporting the care provided by 13 subspecialty programs that act as tertiary/quaternary referral programs, only three of which have coordinator-level support. GI patients and their families are inherently more complex due to the conditions seen that require dedicated nursing expertise, as well as a frequent component of functional pain that is not easily amenable to routine interventions but requires psychological support and guidance to families by RNs.

This triage nurse position covers pediatric gastroenterology outpatient patient care needs.
• Managing heavy volume of patient calls and electronic medical record communications between providers and families, including high acuity sick calls and triage to appropriate member of the health care team in a timely manner.
• Provides recommendation to patient/family member regarding care based on information obtained in triage process, collaborating as necessary to establish and coordinate the plan of care, while anticipating patient care needs across the continuum.
• Utilizes triage skills and practices to assess patients’ needs and advise appropriately, and documents patient information and findings in EPIC.
• Continually utilizes the triage and nursing process to evaluate patient's progress towards goals by applying appropriate interventions to ensure effective outcomes.
• Support families by writing letters, filling out school forms and FMLA paperwork.
• Manage heavy volume of order requests for supplies, medications and procedure requests.
• Support teammates and co-workers when out sick or on vacation.


JOB SUMMARY
Applies the nursing process to plan and implement the care of patients, including patient and family education and continuum of care planning. Effectively communicates pertinent patient/family information to the health care team. Collaborates in establishing patient goals and evaluates progress to ensure effective outcomes. Furthers professional growth by engaging in learning activities according to the Board of Nursing standards.

This is a Triage Position
This is an onsite 5 days a week Monday- Friday position
Hours of the clinic are 7:30-5:00 pm

• Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum.
• Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.

Ability to adapt to changes to patient care and area needs with minimal disruption and loss of productivity. Follows up to assure problems and issues are resolved.
