Self Direction Lead Housing Coordinator

Purpose:

The Lead Housing Coordinator is a lead-level role that manages a moderate housing caseload while serving as a point person and support to the Housing team. This role supports people served who receive a Self-Direction housing subsidy in maintaining their housing support through accurate and timely documentation, budgets, and annual recertifications. The Lead Housing Coordinator provides guidance and supervision to Housing Coordinators, collaborates closely with other members of leadership, and works in alignment with other Self-Direction teams, as a support to navigate housing needs of people served. This role also provides education and outreach to people supported and their circles of support around housing options, subsidies, recertification requirements, and ongoing housing responsibilities, and serves as a resource for complex or escalated housing situations, helping identify practical, person-centered solutions.

Requirements

Qualifications:

• Associate’s Degree in a Human Services-related field with two (2) years of experience working with individuals with disabilities preferred; or a High School Diploma with four (4) years of experience working with individuals with disabilities preferred
• Program-specific training (provided)
• Valid NYS Driver’s License acceptable to Agency Insurance standards; must maintain and provide valid insurance if using your own vehicle
• Ability to maintain a professional, kind, courteous, and patient manner at all times
