Position Summary: Collaborates with other members of the healthcare team to provide supportive medical care to clients regardless of their ability to pay by providing nursing care in a mobile clinic or community health ambulatory care setting including health promotion, patient and family support, health maintenance, disease management, coordination of care and maintaining continuity of care.


Responsibilities: *Include but are not limited to

  • Works at outreach and  and various community locations throughout Cleveland
  • Provide care on a mobile clinic to patients who have various social determinants to healthcare
  • Professionally provide treatment to patients who may be homeless, in correctional facilities or other controlled environments
  • Under the supervision of the Clinical Outreach provider perform basic health assessments, vaccinations, injectables and  procedures.
  • Triage patients, schedule follow up care and referrals using EPIC
  • Performs basic health screenings and reports pertinent findings to the management or provider.
  • Implements appropriate components of the plan of care, under the direction of the health care  team. Assure  all needed POCT are completed and documented in EMR prior to provider visit.
  • Is familiar with sterile techniques and setting up of a sterile field.
  • Works collaboratively with other Care Alliance health professionals to facilitate a team approach to the management of its patients.
  • Ability to treat patients in various settings including shelters, behavioral health treatment centers, and outreach events
  • Performs triage/intake on all  patients according to Care Alliance triage/intake procedures in expediting Provider’s daily schedule and patient flow. 
  • Engage, and educate the community on services provided by CAHC.
  • Establish reason for visit. Document patients’ chief complaint and complete all EPIC visit encounter fields and initiate visit note for provider. 
  • Reviews medication list, allergies, pharmacy, and immunizations
  • Complete blood draws, lab work, and assist with women’s health vaginal exams
  • Properly dispose of packages, containers materials, and biohazards as well as expired/wasted medications in appropriate containers.
  • Maintains safe, secure, and healthy work environment by establishing and following standards and procedures: complying with legal, HRSA and OSHA regulations in collaboration with the health care team.


Requirements

 Requirements:

  • HS diploma or Ged and completion of a state approved LPN nursing program.
  • Current license to practice as an LPN in the State of Ohio.
  • Minimum of 2 years’ experience as a LPN, Experience in an community outreach environment preferred
  • Comfortable working with patients in various settings including people who are in outreach clinics, shelters, encampments, and  homeless.  
  • Good problem solving, computer, verbal, and written communication skills.
  • Must be able to efficiently use  EPIC .
  • Current BLS Certification
