2nd Shift LPN/RN Supervisor – Set Monday–Friday Schedule + On Call Every 4th Weekend!

At Heritage Pointe, we’re looking for a skilled and compassionate LPN or RN to join us full-time as a second shift supervisor.  This role offers a set schedule of Monday-Friday from 2p to 10p with on call duties every 4th weekend. 

 

As a successful candidate, you will provide assistance and care for residents, supervise other nursing staff, offer guidance and support, maintain a clean and safe working environment, and ensure that all operational tasks are completed efficiently and on time. This is a GREAT opportunity if you are just starting in the field, or have plenty of experience.

Key Responsibilities:

• Provide skilled nursing care to residents according to their individualized care plans

• Administer medications and treatments with accuracy and attention to detail

• Conduct daily assessments and document changes in residents’ conditions

• Communicate effectively with physicians, residents, families, and team members

• Provide direction and supervision to CNAs and other care staff

• Ensure compliance with all state and federal regulations

Qualifications:

• Current, active RN/LPN license in the state of Indiana

• CPR certification (BLS required)

• Experience in long-term care or geriatric nursing preferred

• Strong clinical judgment and excellent communication skills

• A compassionate, team-focused mindset
