POSITION SUMMARY
This position serves culturally diverse adult and youth residents of the Fruitvale neighborhood of Oakland and its surrounding communities, with an emphasis on local urban Native American and indigenous people. NAHC’s service departments (e.g., Behavioral Health, Medical, and Community Wellness and School-Based) work in close partnership within an integrated system to meet its members’ clinical and non-clinical health needs, including social determinants of health. The focus of this position (>60%) is to provide time-limited clinical behavioral health treatment services for a wide spectrum of age groups and psychiatric presentations, in addition to clinical intakes, assessments, medical service consultation, and crisis management and referral services. The behavioral health team emphasizes quality improvement, clinical effectiveness and efficiency, professional growth, team support, and intercultural proficiency. This position does not have a primary supervisory function but may contribute to supervising a trainee. The BH team currently operates remotely during the COVID-19 pandemic.
JOB DUTIES AND RESPONSIBILITIES
1. Provide medically necessary, service-plan driven, and culturally proficient behavioral health treatment services for people of all age ranges presenting with a broad range of psychological conditions and needs. Service plans are client driven, strengths-based, and consistent with recognized best practices.
2. Conduct clinical intakes and behavioral health assessments in accordance with professional expectations, including biopsychosocial, cultural, diagnostic, and crisis/risk evaluations for the full spectrum of psychological conditions. Includes scheduled appointments; may include “warm handoffs” from other service departments or drop-in services.
3. Provide case management and service linkages (primarily via direct support from team care coordinators), including referrals to supportive resources and if necessary facilitating urgent or emergency service referrals to medical/psychiatric staff/agencies, mobile crisis services, or the police.
4. Create and regularly review service plans that are client-driven, strengths-based, informed by recognized best practices and physical healthcare goals, and adjusted based on clinical need and client priority.
5. Provide clinical consultation/liaison services and education for staff, projects, and departments; includes supporting miscellaneous BH training needs in the agency such as brief consultation for individual providers or didactics for larger groups, if necessary and assigned by supervisor.
6. Document and track service activities in electronic health record systems, paper records, and other data management systems as required in a thorough, accurate, secure, and timely manner, in accordance with agency policy and procedure. Support all evaluation and data reporting requirements.
7. Actively contribute to quality assurance and improvement projects.
8. Ensure maximal quality of services and compliance with the mission and strategic goals of the organization, federal and state laws and regulations, agency policy and procedure, accreditation standards, applicable professional standards, and supervisor/team expectation and/or assigned duties. This includes maintaining current professional knowledge and skill.
9. Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
10. Safely perform all duties; follows required protective protocols to ensure personal safety as well as the safety of others.
11. Work well under pressure in high-volume situations to meet multiple and competing deadlines.
12. Complete other duties as assigned.
