The Senior Case Manager is a professional, clinical registered nurse who assumes responsibility and accountability for the international patient population throughout the continuum of care. In this role, you will facilitate coordination and continuity of care while providing support and education to patients and families. Utilizing advanced nursing skills, you will assess and plan the clinical care of international patients, review patients’ medical conditions with their home country physicians to coordinate care, clarify needs and expectations as well as coordinate transfer of care between international hospitals and UCLA Health.
As our Senior Case Manager, you will be the main point of contact for all international patients and is the primary point of contact for all clinical aspects of patient care, coordinating with multi-disciplinary teams in the outpatient, inpatient, and post-discharge settings. You will collaborate with multi-disciplinary care teams during hospitalization to monitor the overall care experience and to ensure coordination of care across the continuum.
Salary Range: $135,490 to $1778,691 Annually

- Must have BSN degree and RN license.
- Ability to complete a Nursing physical assessment.
- Skill in developing a plan of care including setting goals, monitoring and documenting progress.
- Ability to implement interventions that are pertinent to goals established in the plan of care.
- Ability to evaluate the effectiveness of interventions toward the attainment of established goals and to recognize the need to reassess or reset goal parameters.
- Knowledge base to provide education to patients, families, international referring physicians and other staff regarding clinical and patient care issues.
- Exceptional verbal and written communication clinical skills and expertise in triaging incoming inquiries and referrals with quick turnaround times.
- Ability to collect and relay information with accuracy and diplomacy, and effectively communicate verbally and in writing.
