Utilize your expertise while growing your career as a Community Health Worker Program Supervisor. With guidance and direction from the Senior Director for Care Coordination and Clinical Social Work, the CHW Program Supervisor will provide administrative supervision, program management, and support to a team of hospital-based Community Health Workers (CHWs) in the Emergency Department (ED) at our UCLA Health Ronald Reagan and Santa Monica Emergency Departments. The supervisor will manage and support a team of  up to six (6) CHWs and/or homeless care navigators.  The UCLA Health Emergency Department CHW program will provide navigational, case management, and peer recovery services to patients impacted by social and structural determinants of health, homelessness, substance use, substance use disorder, and/or trauma. Patients may also have significant medical co-morbidities and mental health issues. This supervisory role will coordinate collaboration with the medical home-primary care (PCP), community services, case managers, and social workers, to deliver interventions that will address social needs and maximize patient health outcomes.
