About the role

Responsible for providing care to patients under the direction of the quality leadership.  The Coordinator Clinical Care's primary responsibility is to administer care coordination activities for the patients of medical groups.  This includes monitoring care coordination processes and support primary clinical teams through entering standing orders to facilitate screenings and lab tests as well as engaging patients with preventative and chronic care gaps. It will also include identifying the high priority patient population and working more directly to ensure care coordination for this patient population.  The Coordinator Clinical Care behaves in a professional manner, and consistently demonstrates and promotes the values of Providence.

Required qualifications:

  • H.S. Diploma or GED
  • 2 years of healthcare experience.

Preferred qualifications:

  • Coursework/Training: Completion of an accredited Medical Assistant program and  certified through a nationally recognized Medical Assistant program such as AAMA, AMT, RMA, CCMA or NCCT.
  • Coursework/Training: Completed Nationally Certified Pharmacy Technician or registered with the State Board

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
