Position Overview:
The Triage Registered Nurse - Patient Care Navigator will act as a liaison between patients, their families, and the healthcare team, primarily focusing on guiding patients through the complex healthcare system. This is accomplished by coordinating appointments, educating patients and families about their diagnoses and treatment options, advocating for their needs, and addressing potential barriers to care like transportation or financial concerns. This role will act as a dedicated point of contact to ensure patients receive timely and appropriate care throughout their healthcare journey.
The Triage Registered Nurse - Patient Care Navigator will also provide assistance to patients of DocGo with obtaining referrals, prior authorizations, and necessary treatments/services to ensure comprehensive care is provided. This includes ensuring patients have received and picked up their medications, scheduled diagnostics, and have access to community-based organizations that can address social determinants of health as needed.
Responsibilities:
- Scheduling appointments, coordinating tests, and facilitating communication between different healthcare providers within the patient's care team both with internal and external clinicians and representatives
- Connecting patients with appropriate community resources, support groups, and specialists as needed. Provide assistance to patients for referrals as needed to both clinical and non-clinical services
- Assisting patients with transportation, childcare, housing, and other social determinants of health that may impact their care
- Explaining diagnoses, treatment options, potential side effects, and medication instructions to patients and their families
- Conducting patient assessments to monitor progress and identify potential issues.
