What You'll Do

· Provide skilled nursing care during home visits

· Assess patient needs and help create individualized plans of care

· Administer medications, treatments, and wound care as ordered

· Monitor patient progress and communicate changes with the care team

· Educate patients and families with patience, empathy, and encouragement

· Support patients in maintaining comfort, independence, and quality of life

· Complete documentation accurately and in a timely manner

· Collaborate with physicians, therapists, aides, and other team members to ensure coordinated care

What We're Looking For

· Current and active Registered Nurse (RN) license in Indiana

· Minimum of 1 year of nursing experience

· Home health experience required

· Strong clinical judgment and assessment skills

· A caring, dependable, and patient-centered approach

· Excellent communication and time-management skills

· Current CPR certification

· Valid driver’s license, reliable transportation, and auto insurance
