Rehab PT Field Supervisor

Position Summary:
Rehab Field Supervisor will be responsible for ensuring clinical supervision of staff and ensure the delivery of competent, effective and safe rehabilitation services, including, but not limited to physical therapy, occupational therapy, and speech language pathology.
Rehab Field Supervisor will be responsible for assisting with staff education, training and development. The staff will receive continuing education, in-services and training (in- house and in the field) to further develop and maintain professional competence.


Responsibilities:
• Knowledgeable of CHHA Regulatory guidelines per Center for Medicare and Medicaid, New York State Department of Health (DOH) and Joint Commission (JC).
• Knowledgeable of company policies and procedures as it pertains to clinical staff education and training.
• Assists in conducting orientation and training of the new staff in Extended Home Care’s policies and procedures.
• Accompanies the new staff into the field post orientation, conducts random field visits and complete the annual performance evaluations and field assessments as needed.
• Provides direct supervision of the field staff, including on-site supervisory visits & chart reviews.
• Evaluates and suggests revisions to the existing clinical documentation system.
• Assists in the screening and interviewing process of new organization personnel and makes recommendations for the employment of individuals. Assists in the orientation of new organization personnel.
• Assists the Director of Rehab Services in planning and implementation of in-service training and continuing education programs.
• Participate in McKesson/Netsmart Software Upgrade, Testing and Staff Education.

• Participate in Quality Assurance Performance Improvement (QAPI) activities.
• Participate in the Agency’s Emergency Preparedness Plan.

Qualifications and Experience:

• A graduate of an approved program in physical, occupational, or speech therapy.
• New York State License as a physical, occupational, or speech therapist.
• Current registration from NYS Department of Education in physical, occupational, or speech therapy.
• Minimum of three (3) year of work experience as a physical, occupational, or speech therapist in a medical setting.
• Two years’ experience providing physical, occupational, or speech therapy in the home care setting.
• Previous supervisory experience in the home care department is preferred.
• Demonstrates leadership and strong interpersonal communication skills.

Pay: $115,000 – $125,000 per year
