SUMMARY: A specialized registered nurse who is part of the Wound and Ostomy Department that provides expert care to patients with wounds, ostomies and skin issues. They assess, treat, and educate patients and their families on wound and ostomy care, as well as collaborate with other healthcare professionals to achieve the best outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES –include the following. Other duties may be assigned

Assessment and treatment of inpatients with wound, skin and ostomy care needs.  Works in collaboration with a Wound and Ostomy APRN credentialed at Gaylord Hospital.

  • ·         Evaluate wound and/or ostomy conditions, implement individualized care plans, and provide specialized wound care techniques and treatments.
  • ·         Work closely with Wound and Ostomy APRN and other healthcare professionals to ensure comprehensive patient care.
  • ·         Monitoring and evaluating wound and/or ostomy healing progress, adjusting treatment plans as needed and evaluating the effectiveness of interventions with              the collaboration of the Wound and Ostomy care APRN.
  • ·         Maintains accurate and comprehensive patient documentation, including wound and ostomy assessments, treatments and follow ups
  • ·         Assess and manage stomas, including stoma care and sizing of pouching.
  • ·         Provide care for various types of wounds, including but not limited to pressure injuries, neuropathic wounds, surgical wounds, skin tears, arterial wounds,                        venous ulcers and fistulas.

Provides patient and staff education in regards to wound and ostomy care.

  • ·         Providing patients and caregivers with comprehensive education on wound and/or ostomy care management, preventing further injury, recognizing signs of                   infection and understanding treatment options.
  • ·         Working closely with licensed providers, nurses and other healthcare professionals to ensure a coordinated approach to wound and ostomy care.
  • ·          Provide training and support to licensed providers, nursing staff and other healthcare providers on wound and ostomy care and best practices.

 Research and Innovation:

  • ·         Participating in wound and ostomy care research to improve patient outcomes and stay abreast of advancements in wound and ostomy techniques and                            technologies.
  • ·         Maintain an up to date level of knowledge regarding all aspects of wound and ostomy treatment and management, through continuing medical education, in                order to optimize functioning in the current role.
  • ·         Assists in evaluation of new wound and/or ostomy care products.

 Wound and ostomy department outcomes monitoring

  • ·         Assist in department data collection.
  • ·         Assist in quarterly prevalence and incidence study.
  • ·         Assist in development and maintenance of written policy and procedures for the department.
