Job Title: Licensed Practical Nurse (LPN) – Intake Coordinator (Home Health)

Job Summary:

We are seeking a detail-oriented and compassionate Licensed Practical Nurse (LPN) to join our Home Health team as an Intake Coordinator. In this vital role, you will facilitate the intake and onboarding of new patients, ensure timely processing of referrals, and work closely with clinical and administrative staff to support seamless care delivery.

Key Responsibilities:

• Receive and process home health referrals from hospitals, physicians, and other healthcare providers.
   
• Verify patient insurance, demographics, and home health eligibility criteria.
   
• Communicate with referral sources to gather complete and accurate clinical documentation.
   
• Coordinate start-of-care appointments with clinical staff and schedule initial visits.
   
• Document intake activities accurately in the EMR (Electronic Medical Records) system.
   
• Provide excellent customer service to patients, families, and referral partners.
   
• Ensure compliance with all Medicare, Medicaid, and agency guidelines.
   
• Collaborate with the clinical team to identify any patient needs or concerns during intake.
   

Qualifications:

• Current and active LPN license in the state of CT
   
• Minimum of 1 year of experience in home health, intake, or a related healthcare field (preferred).
   
• Knowledge of Medicare/Medicaid regulations and home health eligibility requirements.
   
• Proficient with EMR systems and standard office software (Microsoft Office, etc.).
   
• Strong organizational, communication, and multitasking skills.
   
• Ability to work independently in a fast-paced environment.
