Job Description:

RN-Case Manager-Full-Time​

• Provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. 
• Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, quality outcomes related to transition of care service delivery and compliance with external review agencies’ requirements
•  The Case Manager supports an environment of caring focused on healing and wellness that is evidence-based.

Requirements:

• Graduate of an accredited school of Nursing
• Current RN license in the state of Kentucky or Kentucky work permit.
• Three to five years-experience required in acute care nursing, case management, utilization management or related field
• Basic Life Support
