PT, OT, or Speech Therapist - Part-time/PRN

Care Plus Homehealth Care is currently hiring Part Time and PRN Physical Therapists, Occupational Therapists and Speech Therapist for the Anderson and Muncie area. Home health services will be provided to patients inside their homes. This position will start out as PRN and there is potential for the home health caseload to grow into a part time position if desired.

• Physical Therapists, Occupational Therapists and      Speech Therapists will provide home health services to clients in      accordance with the State Practice Act for each discipline, Agency policy,      and accepted professional Standards of Practice.

Job Description

1. Provides Physical Therapy, Occupational Therapy, or Speech Therapy to patients under the appropriate guidelines established by each individual client.

2. As requested by the client and in accordance with CPHC guidelines, conducts screening of patients to determine a need for intervention/treatment.

3. Evaluates patients to obtain data necessary for treatment planning and implementation.

4. Interprets and communicates evaluation findings to patients, family and other team members.

5. Establishes individual patient schedules in conjunction with other team members.

6. Develops treatment plans by identifying a problem list, including short and long-term goals and methods to achieve identified goals.

7. Coordinates treatment plan with patient, family and other team members.

8. Implements and conducts treatment outlined in treatment plan.

9. Monitors patient’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.

10. Develops appropriate home or community programming to maintain and enhance the performance of the patient in their own environment.

11. Documents results of patient’s assessment, treatment, follow up and termination of services.

12. Adapts treatment to meet the individual needs of the patient.

Requirements

• Applicants must have an active Indiana license.
• Applicants must have home health experience and      minimum of 1-2 years of experience as a therapist.
• Applicants must have Bachelor's of Science or      Master's Degree in PT, OT or Speech Therapy.
• Applicants must be legally eligible to work in the      United States. Proof of eligibility will be required upon offer of      employment.
