We’re looking for a warm, organized, and proactive Social Services Assistant to support our residents through life’s transitions and ensure they feel heard, supported, and empowered.

- Assist with admissions, discharges, and care planning
- Provide emotional and practical support to residents and families
- Coordinate services and resources to meet individual needs
- Maintain accurate documentation and records
- Be a trusted advocate and friendly face in our community
🎯 What We’re Looking For:
- Experience in a healthcare or post-acute setting preferred
- Social Services Designee Certificate preferred
- Excellent communication and organizational skills
- A heart for seniors and a team-first mindset
