About the role
Overview
The Patient Placement Coordinator is responsible for coordinating bed assignments for all patients admitted or transferred to designated Cone Health facilities in accordance with established policies and bed allocation criteria. The Patient Placement Coordinator facilitates bed assignments for in-house transfers, direct admissions, and for patients originating from the ED, PACU, and other procedural areas. The goal of this position is to promote optimal patient flow. The schedule for this role is 3 - 12 hour night shifts-Wed, Thu, Friday 6p-6a /rotate every 6th weekend (Sat & Sun) 1p-9p for a total of 36 hours a week.
Responsibilities
- Demonstrates knowledge of bed allocation criteria and capacity management system used to place patients. Coordinates bed assignments based on clinical diagnosis, physician specialty and established bed allocation criteria and bed availability. Collaborates with nursing units, physicians, physician?s office staff, interdisciplinary teams, outside facilities, to obtain pertinent patient information to facilitate patient placement. Promptly assigns and notifies nursing units of admissions and in-house transfers by computer message, telephone, or via pager. Manages Emergency Department track board for admission orders, and completes bed request. Assists with paging TRH (hospitalist) for consults at all Cone Health campuses - excluding Alamance Hospital. Assign TRH to patients outside of Cone Health-ex., patients coming from other hospitals and medical offices. Assist Patient Placement RNs with maintaining TRH patient logs and assuring all patients are on teams. Consults with Patient Placement RNs as needed for clinical questions regarding proper patient placement. Responds accurately, pleasantly and professionally to inquiries and requests from nursing units, other departments and outside facilities. Coordinate expirations for body release, autopsies and Medical Examiner cases. Facilitates organ donor process. Reports census information in conjunction with Service Response Center.
- Communicates with Environmental Services to prioritize room-cleaning assignments in order to expedite patient admissions and transfers. Maintains professional manner with changing census demands. Maintains a professional and courteous demeanor when conversing with others. Consistently strives to be an effective and productive member of the department and responds positively to change. Knowledgeable of hospital safety policies infection control policies, departmental disaster plan, and computer downtime procedure. Maintains confidentiality of patient and hospital information. Maintains accuracy in all phases of the job and uses good judgment in the placement of patients. Has a good working knowledge of medical terminology.
