Requirements

• Conduct comprehensive psychiatric evaluations and regular follow-up visits for residents in skilled nursing and post-acute care environments.

• Develop and adjust personalized treatment plans that meet each patient’s unique mental health needs.

• Prescribe, monitor, and manage psychotropic medications, ensuring safety and therapeutic effectiveness.

• Work collaboratively with an interdisciplinary care team —including physicians, nurses, psychologists, and social workers—to coordinate holistic care.

• Provide therapeutic guidance, counseling, and education for patients and families to support treatment adherence and understanding.

• Maintain accurate and compliant documentation in the electronic health record (EHR) system.

• Participate in team meetings, case conferences, and quality improvement initiatives to promote best clinical practices.
