The Embedded Licensed Clinical Social Worker (LICSW) will be an integral part of the healthcare team, working within the primary care setting to provide comprehensive social work services. The LICSW will address the psychosocial needs of patients, offering support, resources, and interventions to improve their overall well-being. This position will be working alongside our Mobile Health Team providing care for primarily unhoused populations. This role involves close collaboration with medical providers, behavioral health specialists, and other healthcare professionals to ensure holistic patient care in the community.

- Will be providing clinical care in non-traditional settings
- Conduct psychosocial assessments to identify patients’ needs, strengths, and barriers.
- Develop and implement individualized care plans in collaboration with patients, families, and the healthcare team.
- Provide brief, solution-focused therapy and crisis intervention.
- Facilitate patient access to community resources, such as housing, financial assistance, and social support services.
- Conduct follow-up to ensure patients’ needs are met and care plans are adjusted as necessary.
- Complete Diagnostic Assessments and Comprehensive Assessments for patients needing Access to treatment services
- Maintain accurate and timely documentation in the electronic health record (EHR) system.
- Ensure compliance with all relevant laws, regulations, and FQHC policies.
- Participate in quality improvement initiatives and data collection to enhance service delivery.
- Work and document with compliance with all deliverables associated with grants received.
- Work closely with primary care providers to integrate behavioral health services into routine medical care.
- Participate in interdisciplinary team meetings to discuss patient care and develop comprehensive treatment plans.
- Serve as a resource for staff regarding social work practice, psychosocial issues, and community resources.
- Serve as consultant for MAT program

Supervisory Responsibilities
This position does not have any supervisory responsibilities.
Work Environment
Primary environment is home office, administrative office, or clinical office. This role requires regular walking. This role may come into contact with patients who may have contagious illnesses.
Physical Demands
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift 15 pounds at times.
Working Conditions: The Embedded LICSW will primarily work our unhoused population and will have to be comfortable with mobile work and outreach activities in the community.
Travel Requirements
Must have the ability to travel between MCC sites in the St. Paul/Minneapolis metro area.
