We hope you will apply to become part of our dedicated, fast-moving team of superstars! ZoomCare is seeking a Medical Director of Primary Care to join our team! The Medical Director of Primary Care is responsible for leading clinical efforts across ZoomCare’s primary care operations, driving performance in key clinical quality metrics, Hierarchical Condition Category (HCC) coding recapture, and certification as a Patient-Centered Medical Home (PCMH). This leadership role requires a dynamic individual who thrives in a collaborative environment and is passionate about improving patient outcomes, optimizing clinical processes, and supporting primary care physicians and teams. - Lead efforts to achieve and maintain optimal clinical quality outcomes across primary care, with a particular focus on preventive care, chronic disease management, and population health.
- Develop and implement strategies to improve performance on key quality metrics (e.g., HEDIS, STAR Ratings, preventive screenings, and chronic care management).
- Monitor and report on clinical quality data, providing actionable insights to clinical teams and leadership to drive improvements.
- Collaborate with operational leadership to ensure seamless integration of clinical initiatives with day-to-day practice workflows.
- Oversee the training and implementation of the process of HCC coding recapture for the primary care population, ensuring accurate documentation and coding practices that reflect the full complexity of patient care.
- Train and support clinical teams on proper coding techniques to optimize risk-adjustment and reimbursement.
- Analyze coding trends and identify opportunities for improvement, while working with clinical and administrative teams to ensure compliance and accuracy.
- Co-lead the initiative to achieve and maintain PCMH certification, working closely with the operations and administrative leadership to align clinical and non-clinical processes.
- Develop and implement strategies to enhance care coordination, patient engagement, and care team collaboration in line with PCMH principles.
- Ensure that clinical workflows are optimized for patient-centered care, emphasizing access, continuity, and comprehensiveness.
- Act as a champion for continuous quality improvement, fostering a culture of learning and innovation within the primary care teams.
- Lead clinical initiatives to streamline operations, reduce inefficiencies, and enhance patient satisfaction.
- Stay current with healthcare trends, regulatory changes, and industry best practices, implementing best-in-class clinical approaches.
- Provide thought leadership and guidance in transforming ZoomCare’s primary care services, driving the adoption of new technologies, workflows, and patient care models.
- Work closely with the Chief Medical Officer, Clinical Operations, and other leaders to develop and implement long-term strategies for clinical excellence.
- Mentor and support primary care providers, providing coaching and guidance on clinical decision-making, patient management, and quality improvement initiatives.
- Serve as a key representative of ZoomCare’s clinical leadership team in meetings, initiatives, and external engagements.
- Share on-site supervision of new providers in their first 3 months (side by side) duties with practice leads
- Serve as the Practice Lead for Family Medicine
- Primary Care Quality Report and Quality Measures
- Weekly one on one meetings with Chief Medical Officer
- Other duties as assigned.
QUALIFICATIONS
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board certification in Family Medicine, Internal Medicine, or a related specialty.
- 5+ years of clinical experience in primary care, with 1+ year in a leadership role preferred.
