Clinical Pharmacist

The clinical pharmacist will be a unit-based pharmacist that will verify the orders and provide clinical services for the area(s) for which they are assigned. They will be a drug information resource for the nurses, physicians, patients, and families.
- All general responsibilities of a Staff Pharmacist.
- Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
- Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions.
- Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution.
- Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management.
- Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings.
- Required:
- Education: Pharm. D. or BS in Pharmacy
- Experience: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
- Licensure/Certification:
- Pharmacy license
- BLS within 30 days of hire (ACLS and/or PALS if code attendance required);
- Board Certification in Pharmacotherapy (BCPS)
