POSITION SUMMARY:

Under the general direction of the Program Director, the Mental Health Therapist will provide individual, family, group and multi-family counseling to family preservation children and families on-site or in the field in accordance with the Department of Child and Family Services (DCFS) contract guidelines and Pacific Clinics’ core values of consumer empowerment and culturally competent service delivery. The Mental Health Therapist will carry between eight to twelve clients/families based on program needs, and perform upfront assessment when assigned.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provides direct clinical services weekly to children and their families.

• Conducts Comprehensive Assessments.

• Develops strengths-based, family-focused, culturally competent, treatment plans.

• Assists in monitoring the safety of children in their home or relative caregiver placement.

• Arranges referrals and linkages to appropriate services and activities for clients.

• Participates in Multidisciplinary Case Planning Committee (MCPCs), staffing and Team Decision Meetings (TDMs) as required.

• Provides transportation to the client on an as needed basis to facilitate service delivery.

• Provides teaching and demonstrating or substitute role model services as needed.

• Coordinates with members of staff multidisciplinary team, Children Social Workers (CSWs), schools and community care providers to ensure continuity of care.

• Assists with consumer crisis during work hours and after hours.

• Documents services provided in accordance with the Clinics’ policy and DCFS contract requirements.

• Provides counseling services to clients; individual adults and/or children, couples, families, or groups. Counseling is face-to-face meetings/interventions the goal of which is to:

• Identify and assist in solving family problems.

• Identify substance abuse and medical mental health issues and refer for treatment.

• Address and treat domestic violence or anger management issues.

• Help to identify personal, vocational and educational goals. Additionally, depending on the education and experience of the counselor more serious interpersonal and dysfunctional family issues may be explored.

• Performs Up-Front Assessments and TDMs for DCFS referrals as requested by the DCFS.

• Models Pacific Clinics’ approach, mission and core values in all communication and correspondence.

• Communicates effectively in a culturally competent and diverse consumer population and promotes favorable interaction with managers, co-workers and others.

• Performs other duties as assigned.

• Reports to work on time and maintains reliable and regular attendance.
