KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across the state of Kentucky and potentially contiguous states. The goals of the Home-Study Post Release Services Program (HS/PRS) are to:
The Caminos Post Release Services (PRS) Clinical Case Manager (CCM) provides community-based
individual and family therapy to recently migrated children, adolescents, and their families.
Clinician focused on personalized, non-insurance-based support
The CCM is responsible for delivering culturally competent services that consist of mental health assessments, screening for human trafficking concerns, crisis intervention and support services and
psychoeducational support groups for recently migrated youth as they adjust to life in the United
States.
The CCM supports the delivery of KYUMH’s trauma responsive model by collaborating with
the Post Release Services case management team to ensure the youth and their caregivers receive
comprehensive services and are connected to both formal and informal resources in their community.

KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across the state of Kentucky and potentially contiguous states. The goals of the Home-Study Post Release Services Program (HS/PRS) are to:
- Assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor’s care (Home Study).
- Provide case management support to ensure the continued safety and stability of children in sponsor’s homes and linking children to resources in the community (Post Release Services).
- Current LPCA, LPCC, MSW, LCSW, LMFTA, LMFT, LMHC OR Eligible for licensure at the time of application.
- Master’s Degree in Social Work, Psychology, Sociology, Counseling, or other relevant behavioral science in which direct clinical experience is a program requirement OR
- Bachelor’s degree in one of the above categories from an accredited college or University with at least five (5) years of clinical experience
- Fluency in both English and Spanish, oral and written
- Experience working with diverse populations, including refugee and/or unaccompanied immigrant children and adolescents
- One-year experience providing direct services to youth and families
- One-year experience delivering community based and / or in-home services
