As a Community Support Specialist, you will provide essential mental health services tailored to the unique needs of children, adolescents, and adults. Collaborating with clients and various stakeholders, you’ll develop and implement individualized treatment plans designed to help clients achieve their goals and flourish in their environments.

Key Responsibilities: 
• Provide community-based mental health services, including assessment, diagnosis, and treatment planning. 
• Collaborate closely with clients, families, and stakeholders to shape and execute individualized treatment plans. 
• Employ a strengths-based approach, transforming lives through empowerment and skill-building using evidence-based practices. 
• Offer behavioral interventions through techniques such as motivational interviewing, operant conditioning, and group dynamics. 
• Coordinate care with primary care physicians, psychiatrists, schools, and community organizations for holistic support. 
 Maintain accurate records of client interactions, treatments, and progress as per organizational policies. 
 Participate in training and development activities to enhance your skills related to mental health services. 
• Work in tandem with other team members to ensure effective care coordination for clients. 
• Develop relationships with key stakeholders, including parents, teachers, and counselors. 
• Prepare and facilitate PSR group sessions as per treatment plan goals.
