We are looking for a Behavioral Health Outpatient Therapist and Social Worker to join our team. This role is brand new, and we are looking for a caregiver that would be up for the challenging yet rewarding work. The Social Worker on this team with work with pregnant and post-partum women at our OB/GYN - Women’s Health Institute. The ideal candidate for this position would be a social worker with a LISW licensure who has both infant/maternal health experience as well as behavioral health experience.
A caregiver in this position works days, 8:00am—5:00pm. No weekends and no holidays.
A caregiver who excels in this role will:
Utilize specialized clinical knowledge and advanced clinical skills in the area's psychotherapy, clinical counseling, diagnostic interviews, assessments, and treatment plans to patients.
Participate in development and implementation of an appropriate assessment based interdisciplinary treatment plan.
Consult with team managers, statutory and voluntary agencies, and patient relatives to ensure compliance with patient treatment plan.
Complete comprehensive psychosocial assessment participates in interdisciplinary rounds.
Provide individual, group, family, and crisis counseling.
Act as a liaison with patients, families, community support workers and facilities to obtain information and facilitate discharge planning.
Provide education and resources for patients and family members.
Supervise and/or train lower-level social workers, students, or interns.
Process paperwork, telephone calls and complete clinical documentation.
Participate in continuing professional education, departmental meetings, and teaching.
Minimum qualifications for the ideal future caregiver include:
Master's degree in social work, counseling, or related field.
Licensed Independent Social Worker (LISW), or Licensed Professional Clinical Counselor (LPCC), or Licensed Professional Counselor (LPC), or Licensed Social Worker (LSW); no record of substantiated ethics or patient abuse reports.
