As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician and the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.</span><span data-ccp-props="{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}">&nbsp;</span></p> <p><strong><span data-ccp-props="{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}">What You'll Do:</span></strong></p> <ul> <li><span data-contrast="none">Manage your assigned patient caseload within your territory and be responsible for the following tasks, including but not limited to: assessing, planning, implementing, and evaluating of patients care throughout all phases of the nursing process.</span></li> <li><span data-contrast="none">Document all notes in the EMR and submit them accurately and in a timely manner</span></li> <li><span data-contrast="none">Communicate and partner cross-departmentally with all attending physicians, hospice staff members, and other agencies as needed </span><span data-contrast="none">to coordinate optimal care for all patients</span></li> <li><span data-contrast="none">Educate and coach all relevant parties on the best practices regarding call levels of care for the patient including: physical, mental, emotional, etc</span></li> <li><span data-contrast="none">Partner with clinical leadership to manage/ad
