Home Health Aide 2 The Home Health Aide/CNA provides assistance with daily living and personal care services in patients homes in accordance with an established care plan and under the supervision of a Registered Nurse or other designated health care professional.

               

  • Traveling to patients' homes to provide assistance with personal care, hygiene, and activities of daily living.
  • Encourages client participation in activities to the extent to which the client is able.
  • Assists with ambulation as instructed/needed.
  • Reports any changes in patient’s condition, living conditions etc. to RN/Supervisor as they occur.
  • Complies with all company policies and procedures including, but not limited to HIPAA Privacy rule
  • Follows care plan as instructed.
  • Accepts only those assignments for which they are qualified.
  • Maintain patients' care records and document provided services.

Required Qualifications

CPR certification

  • This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
  • Excellent customer service
  • Strong clinical background
  • Strong sense of engagement to be a team player
  • Strong written and verbal communication skills
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences
  • Must provide evidence of formal training and/or certification as a Home Health Aide/CNA as required by state law.
  • Must be able to communicate effectively in English, both verbally and in writing. Additional languages preferred but not required.

Preferred Qualifications

  • High School Diploma or GED
  • Knowledge of primary care medicine requirements (state specific).
  • Working knowledge of clinical operations
  • Electronic Medical Record (EMR) experience
  • Previous RN or Medical Assistant experience
  • Ability to type 25-30 words per minutes accurately
