Hoag Compass is a physician-led longevity-focused lifestyle and wellness membership program for patients looking to optimize their health, leveraging the latest in preventative best practices across clinical diagnostics. Compass is a multi-tiered program that includes both hybrid in-person/virtual options and fully virtual pathways, using in-house built digital experiences. Along with its focus on leading edge preventative wellness approaches, Compass also provides patients an integrated care experience via care coordination that helps stitch together their preventative journey with specialty care needs seamlessly. The Hoag Compass Physician will play a key role in representing and delivering care under this clinical philosophy at a pivotal moment in the program’s growth.


Position Details: We are a high-volume practice seeking an energetic self-starter who can work independently and contribute to a five-star service environment. 

  • Full-Time opportunities in Newport Beach, CA, Monday – Friday, and San Clemente, CA, Monday – Friday
  • Be a thought leader in the clinical care philosophy areas crucial to the program and help cascade this philosophy to the supporting care team:
  • Experience applying longevity-oriented interpretation of diagnostics/labs across age cohorts (to help patients with optimization)
  • Experience with lifestyle medicine-based interventional approaches across nutrition, exercise, sleep, and mental health to construct highly personalized care plans to drive towards that optimization
  • Experience with the expanded diagnostics being leveraged within the program 
  • Responsible for managing a limited patient panel through the care lifecycle including health baselining, curating individualized preventative care plans, acute care, chronic care, coordination of care and overall wellness, and regular re-testing as part of this ongoing optimization
  • Conduct visits in-office and virtually via telehealth modalities as well as communicate with patients asynchronously via secure communication platforms
  • Prescribing or administering medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury.
  • Collaborate with the digital team to ensure that all digital experiences and products are safe, effective, optimized for physicians in mind and are patient-centered.
  • Lead a multidisciplinary care team of nutritionists, exercise physiologists, health coaches to support patients who may require deeper dives and help with their recommended lifestyle interventions
  • Partner with business leads to effectively communicate product vision to clinical team
  • Ensure that digital products comply with all relevant regulations and standards including HIPAA and other data privacy requirements.
  • Stay current on the latest trend and technologies in digital healthcare 
  • Stay current on the latest information within the clinical realm of longevity-oriented diagnostics and therapeutics

Requirements

 Qualifications Required:

  • M.D. or D.O. Degree  
  • Board Certified or Board Eligible in Family Medicine or Internal Medicine  
  • EMR proficient (we use EPIC)  
  • Multilingual skills are preferred.  
  • Background in Preventative Precision Medicine and/or Lifestyle Medicine  
  • IFM certification optional
  • Experience with a membership or subscription model is preferred.
