Description
Lead Population Health and Integrated Care Across Primary Care Healthcare Home ServicesThe Assistant Senior Director – Primary Care Healthcare Home is a senior leadership role within Compass Health Network’s Primary Care Healthcare Home (PCHH). As a member of the PCHH Leadership Team, this position provides strategic and operational oversight of Regional Nurse Directors and dietician services, while advancing population health, wellness, and care coordination efforts across the Health Home department. This role plays a critical part in strengthening integrated care, improving outcomes, and supporting consistent, high-quality nursing care management.
Hybrid position, open to Missouri residents with travel to various Compass locations to provide nursing support in clinics, quarterly and as needed for other meetings across the state required.
This is a great opportunity for someone who enjoys leading multidisciplinary teams, using data to drive population health strategies, and shaping systems that support whole-person care in primary care and integrated healthcare settings.
Key Responsibilities
- Provide leadership, supervision, and professional development to Regional Nurse Directors and dietician services
- Strengthen onboarding, training, and competency processes for nursing care management and wellness staff
- Champion population health, quality improvement, and integrated care initiatives across Health Home services
- Review dashboards, reports, and workflows to identify opportunities for operational and clinical improvement
- Support staffing coordination, caseload coverage, and continuity of services during vacancies or leave periods
- Collaborate with Compass treatment teams and Health Home partners to ensure coordinated, patient-centered care
- Partner with senior leadership on process reviews, program optimization, and referral pathways
- Guide health promotion, wellness education, and chronic disease management initiatives
- Ensure compliance with state Healthcare Home guidance, program standards, and reporting requirements
Requirements, Skills, Knowledge and Expertise
- Associate degree in nursing required
- Bachelor’s degree in nursing preferred
- At least two (2) years of supervisory experience required
- Experience in the primary care or behavioral health field preferred
- RN licensed in Missouri required
- Enjoy leading nursing and wellness teams across multiple locations or regions
- Are energized by population health strategies and closing care gaps through data-informed decision making
- Have experience in primary care, behavioral health, or integrated care models
- Thrive in collaborative environments that connect clinical care, operations, and education
- Are motivated by improving access, engagement, and long-term health outcomes for the communities served
