Location: Lexington, KY (Serves Fayette county)

Schedule: Monday-Friday 8:30am-5pm; FT-Temporary Position 

Note: this will be a 3-year grant funded position

As a Bluegrass Care Navigators team member working in the Older Adults Home Modification Program, you will work with individuals in their homes in Fayette County, assessing functional status and home safety, educating, and ultimately enabling low-income seniors to remain in their homes through coordination of low-cost, high impact home modifications which will optimize the home environment and increase independence.

About the role:

The Home Modification Program Occupational Therapist provides client-focused interventions through the Older Adult Home Modification Grant Program to adapt the home environment of eligible low-income senior adults in order to increase independence, promote health and prevent further decline or injury. Assesses the client’s ability to do the things the client wants and needs to do. Recommends grant-funded home modifications as well as assistive devices and adaptive equipment, with the intent of maximizing functional potential and safety for the participant.  

Requirements

• Must have a bachelor’s degree, master’s degree, or doctorate in Occupational Therapy from an institution accredited by the Accreditation Council for Occupational Therapy Education or the American Occupational Therapy Association. 
• Must have passed the NBCOT OTR examination. 
• Must be licensed in the state of Kentucky as an Occupational Therapist. 
• Possesses and maintains current CPR certification. 
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage. 
• 3 years’ experience preferred, a minimum of 2 years’ experience with geriatrics required. 
• Additional experience providing care within the client’s home setting is a plus.  
• Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
