We are a Pay Per Visit compensation model. Pay Per Visit rates range from $70/visit - $140/visit commensurate with experience and depending on type of visit.

As a Home Health Physical Therapist (PT),you will play a vital role in providing care to our patients in various communities in San Diego County.  You will work closely with physicians, to deliver prescribed physical therapy services to patients in the comfort of their own homes.

In this position, you'll travel throughout the day to patients' homes, delivering personalized physical therapy services. You'll use our provided electronic device (smartphone) for documentation, ensuring accurate and up-to-date records.

Your responsibilities will include:

• Providing treatment as ordered by the attending physician, dentist, or podiatrist
• Assisting in developing and updating the plan of treatment or Plan of Care (POC) in an electronic medical record system
• Supervising 1 to 2 Physical Therapy Assistants, ensuring productivity standards are met
• Evaluating patients' level of function and reporting findings to the attending physician and case manager
• Advising, consulting, and instructing family members and Lorian personnel on patients' therapy program
